Pregnant women and individuals with G6PD deficiency should also avoid MB. G6PD deficiency is the most common enzymatic disorder in humans. It is caused by a deficiency of glucose-6-phosphate dehydrogenase, an enzyme present in red blood cells. The most common symptoms are jaundice and hemolytic anemia.

- Nursing mothers: Methylene blue likely passes into breast milk. There is insufficient evidence to confirm that MB is safe for infants, so breastfeeding mothers are advised not to take this risk.
- Individuals with glucose-6-phosphate dehydrogenase (G6PD) deficiency: MB can cause hemolytic anemia in people with G6PD deficiency. Some researchers suggest that G6PD deficiency should be a contraindication for the use of methylene blue. MB must be used with caution in G6PD deficiency. People taking antidepressants should also avoid it, as the combination can dangerously elevate serotonin levels.
